Clinical Report: Late Emergence of Ablation Margin Following CT-Guided Microwave Ablation

Overview

This case study highlights the delayed emergence of the ablation margin following CT-guided microwave ablation of a solid lung nodule. Despite adequate energy delivery, the expected circumferential increased-attenuation margin was not observed until 30.5 hours post-procedure.

Background

CT-guided thermal ablation is a recognized local treatment for lung tumors, particularly for patients who cannot undergo surgery. Guidelines recommend a circumferential increased-attenuation margin of 5–10 mm as an indicator of adequate treatment volume.

Data Highlights

No circumferential increased-attenuation margin was visible on imaging performed 21 minutes after the initial ablation cycle. The margin only became evident at 30.5 hours post-procedure.

  • The ablation was performed on a metastatic solid nodule using a single antenna at 45 W for a total of 8 minutes.
  • No circumferential increased-attenuation margin developed during the procedure, despite adequate energy delivery.

  • A sharply demarcated margin with dense consolidation was observed only at 30.5 hours post-ablation.
  • By day 32, most of the consolidation had resorbed, and a cavity had formed.

Clinical Implications

The absence of the guideline-defined margin during the standard observation period should not automatically lead to additional ablation.

Conclusion

This case underscores the importance of recognizing that the expected ablation margin may not appear within the conventional timeframe.
