Clinical Report: CD19 CAR T Therapy Tested in Refractory Rheumatoid Arthritis

Overview

A single infusion of CD19 CAR T cells reduced disease activity in six patients with severe, treatment-refractory rheumatoid arthritis. Follow-up periods ranged from 36 to 52 weeks.

Background

Rheumatoid arthritis (RA) is a chronic inflammatory disorder that can lead to significant morbidity and disability. Traditional treatments often fail in patients with refractory disease.

Data Highlights

ParameterResult
Median disease activity score reduction at week 2449%
Median disease activity score reduction at latest follow-up34%
Participants achieving clinical remission4
Participants maintaining remission without medication3
Decline in anti-mutated citrullinated vimentin antibodies94%

Key Findings

  • All six participants experienced a reduction in disease activity scores.
  • Three participants maintained remission without disease-modifying drugs or glucocorticoids.
  • Grade 1 or 2 cytokine release syndrome occurred in all participants but resolved after treatment.
  • Four participants became seronegative for rheumatoid arthritis-associated autoantibodies.
  • No severe adverse events were reported during the study.

Clinical Implications

Further research is needed to evaluate the long-term safety and efficacy of CD19 CAR T cell therapy in patients with refractory rheumatoid arthritis.

Conclusion

This initial trial indicates that CD19 CAR T therapy may reduce disease activity in severe rheumatoid arthritis.
