Clinical Report: Real-Time Visualization Techniques for Nasoenteral Tube Insertion

Overview

This multicenter study evaluates the feasibility and learning characteristics of real-time visualization-assisted tube placement (VATP) for postpyloric feeding in critically ill patients.

Background

Postpyloric feeding is recommended for ICU patients with gastric feeding intolerance or high aspiration risk. Timely and accurate tube insertion is critical for effective enteral nutrition. Traditional methods may be hindered by the need for specialized equipment and expertise.

Data Highlights

MetricValue
Total Procedures316
Successful EDS Procedures294
Radiographic Confirmation292 (94.8%)
Median Procedure Duration25 min (IQR, 15–37.5)
Success Rate (Operators)82.4% to 100%
Distal Placement (D4 or beyond)199 cases (64.6%)

Key Findings

  • VATP achieved a high overall success rate of 94.8% for postpyloric tube placement.
  • Median procedure duration decreased from 27 min in the early phase to 20 min in the late phase.
  • Operator-specific success rates varied, with 8 of 10 operators achieving rates above 90%.
  • Distal placement at or beyond D4 increased from 60.5% to 72.2% as operator experience improved.
  • Minor adverse events were reported, but no severe complications occurred.

Clinical Implications

The findings indicate that VATP can be integrated into ICU practice for postpyloric tube placement.

Conclusion

VATP represents a method for postpyloric tube placement in critically ill patients, with outcomes related to procedural success and efficiency as operator experience increases.
