Clinical Report: Impact of Nd: YAG Laser Capsulotomy on Central Macular Thickness

Overview

This systematic review and meta-analysis evaluated changes in central macular thickness (CMT) following Nd: YAG laser capsulotomy for posterior capsule opacification (PCO). Significant increases in CMT were observed at postoperative week 1, month 1, and month 3, with no change at day 1.

Background

Posterior capsule opacification (PCO) is a common complication after cataract surgery, affecting visual acuity. Nd: YAG laser capsulotomy is the standard treatment for PCO, but it carries risks of complications, including cystoid macular edema (CME).

Data Highlights

Time PointMean Difference (MD) μmp-value
Day 10.060.970%
Week 18.72< 0.00185%
Month 13.69< 0.00112%
Month 35.07< 0.0010%

Key Findings

  • Significant increase in CMT at week 1 (MD: 8.72 μm; p < 0.001).
  • No significant change in CMT at day 1 (MD: 0.06 μm; p = 0.97).
  • At month 1, CMT increased by 3.69 μm (p < 0.001).
  • At month 3, CMT increased by 5.07 μm (p < 0.001).
  • Greater CMT changes at week 1 were associated with laser energy ≥ 50 mJ and baseline BCVA ≥ 0.5 logMAR.
  • No significant associations found in meta-regression for ethnicity, study design, steroid use, or sample size.

Clinical Implications

Monitoring CMT in the postoperative period is necessary.

Conclusion

This meta-analysis provides evidence of significant CMT changes following Nd: YAG laser capsulotomy.
