Clinical Report: Insights and Prognostic Evaluation of Bladder Paraganglioma

Background

Bladder paraganglioma is a rare neuroendocrine tumor, accounting for only 0.3%–1.0% of all paragangliomas. Its clinical symptoms are often nonspecific, leading to a high rate of misdiagnosis. Early diagnosis and appropriate management are crucial to minimize surgical complications and improve prognosis. The preoperative misdiagnosis rate exceeds 60%.

Data Highlights

No numerical data available.

Key Findings

  • The patient exhibited paroxysmal hypertension during urination, with peak blood pressure reaching 205/120 mmHg.
  • Diagnostic tests revealed elevated plasma levels of norepinephrine and methoxy norepinephrine.
  • Pelvic MRI showed a mass on the right bladder wall with hydronephrosis and suspicious pelvic lymph nodes.
  • Postoperative pathology confirmed bladder paraganglioma invading the full thickness of the bladder detrusor muscle.
  • No lymph node metastasis was observed in the examined lymph nodes.
  • During a 5-month follow-up, no recurrence or metastasis was detected.

Clinical Implications

Preoperative catecholamine metabolite testing and pelvic MRI are essential for diagnosing bladder paraganglioma.

Conclusion

Bladder paraganglioma requires careful diagnostic and surgical planning.
