Guideline-Based Recommendations

Diagnosis

  • Diagnosis of anemia in MF defined by Hb <11 g/dL in men and <10 g/dL in women per 2024 IWG-ELN criteria.

  • Spleen size evaluated by 2013 ELN (IWG-MRT) criteria.

Management

  • Initiate momelotinib at 200 mg daily for MF patients with anemia and symptomatic splenomegaly.
  • Consider momelotinib for both JAKi-naïve and JAKi-exposed patients, including those with prior cytopenias.
  • Combination therapy with ESAs may be considered to enhance early anemia response.

Monitoring & Follow-up

  • Assess hemoglobin levels and transfusion frequency at baseline, 3 months, and 6 months.
  • Monitor spleen size reduction using ELN criteria.
  • Evaluate symptom response regularly given high baseline symptom burden.

Risks

  • Prior JAK inhibitor toxicity may influence response rates to momelotinib.

154 adult MF patients with anemia; 76.6% previously exposed to JAK inhibitors.

Momelotinib demonstrated anemia improvement and spleen size reduction with median follow-up of 5 months; combination with ESAs showed higher early response rates.
