Clinical Scorecard: Zanubrutinib Alone for CD79B-Mutated Relapsed or Refractory Diffuse Large B-Cell Lymphoma: A Multicenter Phase 2 Trial

Category
Detail
Condition
CD79B-mutated relapsed or refractory diffuse large B-cell lymphoma (DLBCL)
Key Mechanisms
CD79B mutations are associated with chronic active B-cell receptor signaling and constitutive NF-κB pathway activation; BTK mediates B-cell receptor signaling.
Target Population
Adults with histologically confirmed DLBCL, a CD79B mutation identified by validated PCR assay, and disease relapsed or refractory after at least one systemic treatment.
Care Setting
Open-label, single-arm, multicenter Phase 2 trial in China; zanubrutinib was administered orally and patients were followed for response, safety, and survival.
Key Highlights
Among 65 treated patients, investigator-determined overall response rate was 46.2% (95% CI, 33.7–59.0), including a 29.2% complete response rate.
Median duration of response was 22.7 months (95% CI, 2.8–not estimable); median progression-free survival was 4.3 months and median overall survival was 18.1 months.
Activity was observed mainly in non-GCB DLBCL; the small GCB subgroup had no objective responses.
Grade 3 or higher treatment-emergent adverse events occurred in 43.1% of patients; pneumonia was the most frequent Grade 3 or higher event (9.2%).
The study was an open-label, single-arm Phase 2 trial, and the reported biomarker analyses were exploratory.
Guideline-Based Recommendations
Diagnosis
Trial eligibility required histologically confirmed DLBCL and a CD79B mutation identified using a validated amplification refractory mutation system PCR assay.
Management
Zanubrutinib was administered orally at 160 mg twice daily in uninterrupted 28-day cycles until progressive disease, unacceptable toxicity, or study discontinuation.
Monitoring & Follow-up
A safety visit was scheduled approximately 30 days after the final dose, with prespecified safety and laboratory evaluations.
Patients who permanently stopped zanubrutinib remained under long-term survival observation.
Risks
Patient & Prescribing Data
The trial enrolled 65 Chinese adults with CD79B-mutated relapsed or refractory DLBCL. Median age was 66 years; 86.2% had non-GCB disease, and the median number of prior treatment lines was one.
Zanubrutinib was given as monotherapy at 160 mg orally twice daily. Median observation was 13.9 months. The study reported an overall response rate of 46.2%, median duration of response of 22.7 months, and median progression-free survival of 4.3 months.
Clinical Best Practices
The trial assessed investigator-determined response using Lugano 2014 criteria.
The study was open-label and single-arm; activity was observed mainly in non-GCB DLBCL, while the small GCB subgroup had no objective responses.
The evolution of breast cancer care is increasingly challenging a long-standing assumption in oncology: that greater treatment intensity necessarily translates into better outcomes.
