Objective:

To report an unusual case of tonsillar adenoid cystic carcinoma (TACC) with a 47-year disease trajectory.

Key Findings:
  • The patient exhibited a prolonged indolent phase of over 40 years before experiencing rapid disease progression in the last 6 years.
  • Molecular profiling indicated genomic alterations.
Interpretation:

  • Detailed treatment records from the initial diagnosis in 1977 were unavailable.
  • The exact chemotherapy regimen during the 2018 treatment could not be fully retrieved.
Conclusion:

This case represents one of the longest documented clinical courses of TACC.
