Objective:

To report a case of multifocal focal eosinophilic myositis (FEM) with extra-ocular muscle involvement.

Key Findings:
  • The patient exhibited isolated extra-ocular muscle involvement.
  • Tocilizumab treatment led to improvement in diplopia, ocular motility, and reduction in muscle swelling on MRI.
Interpretation:

Extra-ocular muscle involvement in FEM may indicate a more treatment-resistant phenotype.

Limitations:
  • This is a single case study, limiting generalizability.
  • Long-term efficacy and safety of tocilizumab in this context remain to be established.
Conclusion:

Tocilizumab may represent a therapeutic strategy for refractory cases of FEM with extra-ocular muscle involvement.
