Negotiating complexity in spite of scarce evidence—how to care for older ICU patients?

Objective: To address the challenges and lack of evidence in managing older patients in the ICU and to summarize recent guidelines from the SCCM.
Approach: Key Findings: There is a scarcity of evidence and guidance for managing older ICU patients. The SCCM panel issued a conditional recommendation for geriatric models of care, despite low certainty of evidence. No recommendations were made regarding post-ICU follow-up or the use of antipsychotic medications for delirium treatment. Interpretation: The SCCM guidelines acknowledge the limitations of current evidence in managing older ICU patients.
Limitations: The evidence base for managing older ICU patients is limited and often uncertain. Many clinical trials exclude geriatric patients, leading to a lack of applicable data. Conclusion: Further research is needed to identify effective interventions for older ICU patients.
