Objective:

To report a case of cervical disc herniation with myeloradiculopathy in a patient with clinically probable alkaptonuria-related ochronosis.

Approach:
    Key Findings:
    • Intraoperatively, the intervertebral discs and nucleus pulposus were diffusely black-pigmented, suggesting ochronotic involvement.
    • Postoperative improvement in limb numbness and gait stability was observed, as noted in the source.
    Interpretation:

    Limitations:
    • Diagnosis was regarded as clinically probable due to lack of biochemical confirmation.
    • Quantitative urinary homogentisic acid measurement and HGD genetic testing were not completed.
    Conclusion:

Standard cervical decompression and fusion principles are effective for neurologic compromise.
