Objective:

To report a case of coexistence of a subepithelial nevus and a papilloma at the lacrimal caruncle and review existing literature on caruncular tumors.

Approach:
  • Case Presentation: A 63-year-old Thai woman presented with visual loss in her right eye and a hyperpigmented mass on her left lacrimal caruncle. MRI indicated malignant choroidal melanoma in the right eye. Surgical intervention included enucleation of the right globe and excision of the left caruncular mass using the no-touch technique, followed by double freeze-thaw cryotherapy.
Key Findings:
  • The patient had a choroidal melanoma in the right eye and a subepithelial caruncular nevus with an overlying conjunctival papilloma in the left eye.
  • Coexistence of benign caruncular tumors is rare, with only three similar cases reported in literature.
  • Complete tumor resection using the no-touch technique was performed.
Interpretation:

The case highlights the rarity of benign caruncular tumors and the importance of histopathological examination for definitive diagnosis.

Limitations:
  • The study is based on a single case report, limiting generalizability.
  • Long-term follow-up data beyond 12 months is not provided.
Conclusion:

The coexistence of a subepithelial nevus and papilloma at the lacrimal caruncle is rare.
