Objective:

To detail a novel percutaneous approach (the Q2 approach) for implanting peripheral neuromodulation stimulators at the C2 dorsal root ganglion specifically for managing intractable headaches.

Key Findings:
  • All 4 patients achieved >50% pain relief during the 60-day implant period.
  • 3 of 4 patients demonstrated sustained benefit beyond the implant period, with follow-up conducted at 10, 30, 60, and 100–120 days.
  • No complications such as lead migration or infection were observed.
Interpretation:

The Q2 approach is a feasible and safe method for accessing the C2-DRG for headache management, providing a significant alternative to existing treatment modalities.

Limitations:
  • Small sample size of only 4 patients.
  • Short follow-up duration for assessing long-term efficacy, and lack of a control group.
Conclusion:

The Q2 approach offers a minimally invasive option for managing intractable headaches through peripheral neuromodulation at the C2-DRG.
