Objective:

To compare immune cell subsets, cytokine profiles, and inflammatory mediators in axial spondyloarthritis (axSpA) and psoriatic arthritis (PsA), which are overlapping yet distinct conditions within the spondyloarthritis spectrum.

Approach:
  • Study Design: Peripheral blood mononuclear cells (PBMCs) from 179 patients (88 axSpA, 91 PsA) and 49 healthy donors were profiled using spectral flow cytometry.
  • Data Collection: The study involved 230 million acquired events and 55 million quality-controlled cells to analyze immune subsets, activation- and differentiation-associated markers, and cytokines.
Key Findings:
  • AxSpA exhibited an expansion of double-negative and γδ T cells.
  • Increased plasmablasts and CD21low B cells were observed in axSpA.
  • Higher expression of activation-associated markers was noted in axSpA compared to PsA.
Interpretation:

Distinct immunophenotypic profiles were identified between axSpA and PsA.

Conclusion:

The findings highlight the unique immune profiles of axSpA and PsA.
