Objective:

To investigate the associations among choroid plexus (CP) morphology, glymphatic-related fluid alterations, white matter hyperintensities (WMH), and cognitive performance in individuals with type 2 diabetes mellitus (T2DM).

Approach:
    Key Findings:
    • T2DM patients had lower SDMT scores and larger CP volume, FW fraction, PVS volume, and PWMH volume compared to controls (all P < 0.05).
    • CP volume and FW fraction were independently associated with PWMH in T2DM patients.
    • FW fraction and PWMH were independently linked to SDMT scores.
    • Significant mediation effects were found through FW in the CP–PWMH association and through PWMH in the FW–SDMT association.
    Interpretation:

    Limitations:
    • The study is cross-sectional, limiting causal inferences.
    • Sample size may restrict the generalizability of findings.
