Objective:

To report a case of postoperative left common carotid artery dissection after acute type A aortic dissection repair and the use of a hybrid strategy for treatment, emphasizing its significance.

Key Findings:
  • The patient developed severe right-sided hemiparesis postoperatively due to near-occlusion of the left common carotid artery.
  • Emergent surgical exposure allowed for successful retrograde access and deployment of three overlapping self-expanding stents.
  • Final angiography showed complete reperfusion with a modified Thrombolysis in Cerebral Infarction grade of 3 and no intracranial distal embolization.
Interpretation:

Hybrid open retrograde transcarotid access may serve as a viable option for treating postoperative carotid artery dissection when traditional access routes are not feasible, highlighting its importance in complex cases.

Limitations:
  • Limited reports exist on the use of surgical cervical exposure for retrograde transcarotid access, which may carry risks associated with surgical exposure and access-site complications.
Conclusion:

The case demonstrates the effectiveness of a hybrid approach in managing carotid artery dissection following aortic dissection repair, suggesting its potential role in future clinical practice.
