Objective:

To report a case of primary plasma cell leukemia (pPCL) treated with allogeneic hematopoietic stem cell transplantation (allo-HSCT) after combination chemotherapy.

Key Findings:
  • The patient achieved complete remission after chemotherapy.
  • Post-transplantation, bone marrow flow cytometry showed only 0.003% normal plasma cells and complete donor chimerism of 99.91%.
  • The patient experienced long-term disease-free survival after the sequential treatment approach.
Interpretation:

This case provides real-world evidence supporting the potential curative role of sequential chemotherapy followed by allo-HSCT in pPCL.

Limitations:
  • The rarity of pPCL limits generalizability of findings.
  • Long-term follow-up data is limited due to the patient discontinuing regular follow-up.
Conclusion:

The sequential treatment approach of chemotherapy followed by allo-HSCT may offer a viable option for achieving long-term survival in pPCL.
