Fragile self, mechanical world: mechanistic delusions and ego fragility in schizotypal–affective spectrum disorder—a CARE case report
To present a case study of a patient with complex psychiatric symptoms that defy traditional categorical diagnoses, with a focus on a dimensional psychopathological analysis.
Approach:
Patient Presentation: A 44-year-old Brazilian man with systematized persecutory ideation, structural ego fragility, and absent insight, leading to social and occupational deterioration. Prior diagnoses of bipolar disorder and provisional schizophrenia were considered but did not fully capture the clinical complexity.
Key Findings:
The patient exhibited severe dyslipidemia and marked hyperandrogenism, which may be associated with limbic hyperactivation, though causality cannot be established.
Psychometric assessment indicated high levels of suspiciousness and unusual thought content.
Treatment resulted in reduced paranoid reactivity and improved family engagement.
Interpretation:
The case illustrates the complexity of psychotic-affective presentations that do not fit neatly into existing diagnostic categories.
Limitations:
Causality between hyperandrogenism and limbic hyperactivation cannot be established from a single cross-sectional measurement.
