Expert panel endorse individualized beta-lactam dosing for acutely ill patients
International guidance supports concentration-guided dosing for selected acutely ill patients, with clinical cure showing the clearest benefit.

Objective: To evaluate the effectiveness of individualized beta-lactam dosing guided by measured drug levels in critically ill patients.
Approach: Panel Composition: A 19-member international panel of experts in various fields convened to develop recommendations.Evidence Review: The panel conducted a systematic review of research databases and clinical trial registries, screening studies and extracting data.Consensus Development: When evidence was insufficient, expert consensus was used to develop best-practice statements.Key Findings: Patients receiving individualized beta-lactam dosing had nearly twice the odds of clinical cure compared to usual care. The evidence for mortality reduction was inconclusive, with no statistically significant difference found. Individualized dosing did not significantly improve microbiologic eradication or shorten hospital stays. Interpretation: The panel conditionally recommended individualized dosing for critically ill patients and those with altered kidney function.
Limitations: Few randomized trials and differences in outcome definitions. Uncertainty in results and limited access to rapid drug-level testing. Safety evidence remains unclear, particularly regarding high beta-lactam levels. Conclusion: Individualized beta-lactam dosing may improve treatment outcomes.
