Objective:

To report a rare case of a giant communicating solitary fibrous tumor (SFT) involving the temporal skull base and infratemporal fossa, highlighting diagnostic and surgical challenges.

Approach:
    Key Findings:
    • Thetumormeasuredapproximately74×61mmandshowednonecrosisormarkedcytologicatypia.ImmunohistochemistryrevealednuclearSTAT6positivity,CD34,CD99,andBcl-2expression,withaKi-67labelingindexofapproximately5%,supportingthediagnosisofSFT.Postoperativefollow-upat22monthsshowedsymptomimprovementandnorecurrence.
    Interpretation:

This case highlights the importance of thorough clinicoradiological assessment and multidisciplinary surgical approaches in managing complex skull base tumors.

    Limitations:
    • The rarity of the tumor limits the generalizability of findings.
    • Preoperative imaging could not definitively distinguish SFT from other tumors.
    Conclusion:

This case adds to the limited literature on skull base SFTs with intracranial-extracranial extension.
