Daily Itch Duration Linked to Quality-of-Life Burden

Objective:

To evaluate the relationship between disease severity, itch-related measures, and quality of life (QoL) in patients with chronic inflammatory skin diseases.

Approach:
  • Study Design: A prospective, cross-sectional study involving 522 adult patients with chronic pruritus from various dermatology practices in Germany.
  • Assessment Methods: Dermatologists assessed disease severity using disease-specific instruments, while patients completed validated questionnaires on itch intensity, duration, scratching frequency, sleep disturbance, and QoL.
  • Data Analysis: Multivariable regression and mediation analyses were performed to explore the mediating effects of itch-related measures on the relationship between disease severity and QoL.
Key Findings:
  • Daily itch duration was identified as the strongest mediator between disease severity and QoL.
  • Sleep disturbance, scratching frequency, and average itch intensity also mediated the association but to a lesser extent.
  • Impaired QoL correlated more strongly with itch-related measures than with overall disease severity.
  • Female sex, moderate disease severity, scratching frequency, and sleep disturbance were independently associated with greater QoL impairment.
  • Patients with atopic dermatitis, chronic prurigo, and chronic urticaria experienced higher QoL impairment compared to those with psoriasis.
Interpretation:

Limitations:
  • The cross-sectional design limits causal inference.
  • Disease severity was harmonized into a composite measure, which may not fully capture individual disease characteristics.
  • Chronic urticaria severity was assessed using a nonvalidated method.
  • Patient groups were unequal in size.
  • The study only included adult patients, limiting generalizability to pediatric populations.
