Objective:

To compare the efficacy and safety of intravitreal brolucizumab versus panretinal photocoagulation (PRP) in treatment-naïve patients with proliferative diabetic retinopathy (PDR).

Key Findings:
  • Brolucizumab showed a mean gain of 0.2 letters in best-corrected visual acuity (BCVA), while PRP resulted in a decline of 4.2 letters, indicating a significant difference of 4.4 letters.
  • 63.6% of patients treated with brolucizumab had no evidence of PDR at week 54 compared to 22.4% in the PRP group.
  • Rates of center-involved diabetic macular edema (CI-DME) were lower with brolucizumab (31.1%) compared to PRP (72.7%).
  • Overall ocular adverse events were less frequent in the brolucizumab group (34.3%) compared to PRP (49.1%).
Interpretation:

Limitations:
  • Intraocular inflammation, including retinal vasculitis, was observed in 5.2% of brolucizumab-treated patients.
  • The study primarily focused on treatment-naïve patients, which may limit generalizability.
Conclusion:
