To evaluate the effectiveness of ultraviolet-induced fluorescence dermoscopy (UVF-dermoscopy) in differentiating between scarring and nonscarring alopecias.
Approach:
Study Design: A multicentric retrospective observational study analyzing 84 patients with alopecia across five international dermatology centers.
Patient Demographics: Patients included 43 with nonscarring alopecia and 41 with scarring alopecia, categorized by skin phototypes I-III and IV-VI.
Dermoscopy Techniques: Comparison of UVF-dermoscopy and polarized dermoscopy in detecting diagnostic markers for alopecia classification.
Key Findings:
UVF-dermoscopy identified follicular ostia in nonscarring alopecias at a rate of 83.7% compared to 34.9% with polarized dermoscopy (p < 0.01).
Bright white areas were detected in scarring alopecias at 70.7% with UVF-dermoscopy versus 30.1% with polarized dermoscopy (p = 0.01).
In dark skin (phototypes IV-VI), UVF-dermoscopy significantly improved detection of follicular ostia (57.9% vs. 10.5%, p < 0.01), but not bright white areas.
Interobserver agreement for UVF-dermoscopy was higher (Cohen's kappa 0.83) compared to polarized dermoscopy (0.57).
