Case Report: Adebrelimab-associated suspected immune-related myocarditis with myositis/myasthenia gravis overlap syndrome (IM3OS): A rapidly fatal case and clinical implications
To report a case of suspected immune-related myocarditis with myositis/myasthenia gravis overlap syndrome (IM3OS) following treatment with adebrelimab in a patient with intrahepatic cholangiocarcinoma.
Approach:
Key Findings:
The patient experienced rapid onset of severe symptoms, including chest tightness and muscle weakness, 3 days after the second cycle of adebrelimab.
Laboratory tests indicated significant myocardial injury, with elevated markers such as high-sensitivity troponin T and N-terminal pro-B-type natriuretic peptide.
Despite treatment efforts, the patient succumbed to cardiac arrest on day 9 after the onset of symptoms.
Interpretation:
The case highlights the rapid progression and poor prognosis associated with adebrelimab-induced IM3OS, emphasizing the need for early recognition and aggressive management.
Limitations:
The case study is based on a single patient, which limits the generalizability of the findings to a broader population.
Clinical characteristics and optimal management strategies for IM3OS remain poorly defined and require further investigation.
Conclusion:
This case underscores the critical importance of early detection and multidisciplinary intervention in managing severe immune-related adverse events associated with immune checkpoint inhibitors.
