Objective:

To clarify the association between the systemic immune-inflammation index (SII) and post-stroke depression (PSD) in adult stroke patients.

Key Findings:
  • Seven studies involving 2,780 patients were included, with 822 developing PSD.
  • High SII at admission was significantly associated with an increased risk of PSD (OR = 2.14, 95% CI: 1.74–2.64).
  • Sensitivity analyses confirmed stability of findings across various subgroups.
Interpretation:

Elevated SII may be independently associated with an approximately twofold increased risk of PSD.

Limitations:
  • Inconsistencies in previous studies regarding SII and PSD.
  • Limited by small sample sizes and heterogeneous methodologies in included studies.
Conclusion:

SII may serve as a simple and accessible inflammatory biomarker for early identification of patients at higher risk of developing PSD.
