A systematic review analyzed the accuracy of frozen section (FS) biopsy compared to final paraffin-embedded histopathology in 15 studies, revealing high specificity but variable sensitivity influenced by tumor grade, lesion presence, and pathologist experience. While FS biopsy often confirmed malignancy, its sensitivity ranged from 41% to 97%, particularly underperforming in complex atypical hyperplasia and early-stage cancers. The review concluded that FS biopsy serves as a complementary method rather than a definitive diagnostic tool, emphasizing the necessity of final histopathology for accurate endometrial assessment.
