Objective:

To evaluate the effectiveness of scleral lenses in managing exposure keratopathy due to incomplete eyelid closure.

Approach:
  • Case History: A 59-year-old woman with chronic facial nerve palsy and persistent exposure keratopathy was evaluated for scleral lens fitting after prior management strategies failed.
  • Lens Fitting: The patient was fitted with a SynergEyes VS scleral lens and a daily disposable soft lens, with iterative adjustments made to improve fit and comfort.
Key Findings:
  • The patient achieved visual acuity of approximately 20/20 OD with improved comfort and daily function.
  • Corneal integrity improved, with reduced epithelial compromise and more stable ocular surface findings.
  • Scleral lenses provided sustained ocular surface protection.
Limitations:
  • The study is based on a single case report, limiting generalizability.
  • Long-term outcomes and comparative effectiveness with other management strategies are not addressed.
