Objective:

To identify preoperative predictors of contralateral patent processus vaginalis (CPPV) and evaluate sex-specific risk factors in children with unilateral inguinal hernia or hydrocele.

Approach:
    Key Findings:
    • The detection rate of CPPV in children undergoing laparoscopic repair for unilateral inguinal hernia is 38.5%.
    • Younger age and female sex were identified as independent risk factors for CPPV.
    • Age < 1 year and left-sided hernias were significantly associated with CPPV.
    Interpretation:

Preoperative prediction of CPPV risk may inform surgical decision-making and contralateral inspection during laparoscopic procedures.

    Limitations:
    • The study is retrospective and may have inherent biases.
    • Ultrasonography was not routinely performed, potentially affecting the detection of CPPV.
    Conclusion:

Identifying preoperative factors associated with CPPV can enhance surgical planning and patient management in pediatric inguinal hernia cases.
