Objective:

To present a case of endoscopic resection of left side L5/S1 pseudoarthrosis in a 17-year-old girl with Bertolotti syndrome (BS).

• Postoperative pain scores improved significantly: VAS back pain score from 8 to 2, and leg pain score from 8 to 1 at the 3-month follow-up.
• Symptoms of back pain and radiculopathy completely resolved at the 3-month follow-up.

Endoscopic decompression via UBE technique can be an effective surgical option for adolescents with BS after conservative treatment failure.

Limitations:

• The study presents a single case, limiting generalizability.
• Lack of long-term follow-up data beyond 3 months.

Conclusion:

Endoscopic decompression via UBE technique is a promising approach for treating adolescent patients with Bertolotti syndrome.
