Differences in clinical features and prognosis between hypertriglyceridemia and other causes of acute pancreatitis: a dual perspective based on metabolic disorders and inflammatory response
To investigate the clinical features and prognosis of acute pancreatitis (AP) with different etiologies, specifically comparing hypertriglyceridemia acute pancreatitis (HTG-AP) to non-hypertriglyceridemia acute pancreatitis (non-HTG-AP).
Approach:
Key Findings:
HTG-AP patients were significantly younger than non-HTG-AP patients (P < 0.001).
HTG-AP patients had higher body mass index (BMI) and diabetes prevalence (P < 0.001).
HTG-AP patients exhibited higher levels of fasting blood glucose, serum calcium, albumin, uric acid, and inflammatory markers (P < 0.05).
Higher rates of moderately severe acute pancreatitis (MSAP) and severe acute pancreatitis (SAP) were observed in the HTG-AP group (P < 0.05).
The ICU admission rate was significantly higher in the HTG-AP group (P < 0.05).
HTG-AP was associated with a higher incidence of post-acute pancreatitis diabetes mellitus (PPDM-A) after 3 months (P < 0.05).
HTG-AP patients had a lower rate of cardiovascular disease compared to non-HTG-AP patients (P < 0.01).
Interpretation:
HTG-AP presents with more severe metabolic disorders and a stronger inflammatory response compared to other etiologies of acute pancreatitis, leading to higher rates of severe pancreatitis and an increased long-term risk of diabetes, which should inform clinical management strategies.
Limitations:
Single-center study may limit generalizability and introduce potential biases.
Follow-up duration of 3 months may not capture long-term outcomes.
Conclusion:
HTG-AP patients exhibit distinct clinical characteristics and worse outcomes compared to other causes of acute pancreatitis, highlighting the need for tailored management strategies.
