Objective:

To estimate the proportion of patients hospitalized for myocardial infarction (MI) or stroke who receive a new diagnosis of diabetes (NDD) at the time of hospitalization.

Key Findings:
  • 16.0% of patients with MI (95% CI 14.3–17.7) and 15.3% of patients with stroke (95% CI 12.5–18.0) were newly diagnosed with diabetes.
  • Higher NDD rates were observed when using oral glucose tolerance tests compared to hemoglobin A1c alone.
  • The proportion of NDD has not significantly changed over the last 30 years.
Interpretation:

Approximately one in six patients hospitalized for MI or stroke receives a new diabetes diagnosis, highlighting the urgent need for improved diabetes screening practices in this vulnerable population.

Limitations:
  • High heterogeneity in the studies included, which may affect the reliability of the pooled estimates.
  • Potential variability in diagnostic criteria and screening practices over time could influence the findings.
Conclusion:

The consistent rate of NDD among MI and stroke patients over three decades underscores the critical importance of enhancing diabetes screening to prevent cardiovascular events.
