To report a case of a large lower-extremity hematoma as a rare vascular complication following transobturator sling surgery and to highlight the role of interventional radiology in its management.

Case Report: A 72-year-old woman underwent urethrocolpopexy with transobturator sling placement. Postoperatively, she developed a large hematoma, which led to imaging studies and a consultation with interventional radiology.

The patient experienced a large lower-extremity hematoma following transobturator sling surgery, while remaining hemodynamically stable and asymptomatic.
Computed tomography angiography and diagnostic arteriography excluded active bleeding or vascular lesions.
