Case report: Sequential complement inhibition and BAFF/APRIL blockade in progressive IgA nephropathy and IgA vasculitis nephritis: a report of two cases
To report the outcomes of two patients with rapidly progressive crescentic disease treated with sequential complement inhibition and BAFF/APRIL blockade.
Approach:
Patient 1: A 74-year-old woman with rapidly progressive IgA nephropathy received eculizumab, followed by iptacopan and telitacicept, resulting in reduced proteinuria and stabilized kidney function.
Patient 2: A 37-year-old man with IgA vasculitis nephritis underwent similar treatment, showing improvements in proteinuria and renal function.
Key Findings:
Both patients exhibited substantial reductions in proteinuria.
Kidney function stabilized during follow-up for both cases.
The treatment approach involved multi-pathway targeted therapy.
