Objective:

To report a case of significant improvement in paradoxical skin lesions associated with SAPHO syndrome following treatment with upadacitinib.

Key Findings:
  • Initial treatments with etoricoxib and methotrexate were ineffective for skin lesions.
  • Adalimumab treatment led to worsening of cutaneous manifestations.
  • Upadacitinib resulted in marked improvement of skin lesions and symptom relief without adverse events.
Interpretation:

Limitations:
  • This is a single case report and may not be generalizable to all patients with SAPHO syndrome.
  • Long-term effects and safety of upadacitinib in this context remain to be established.
Conclusion:

The findings indicate potential efficacy of upadacitinib in managing paradoxical skin lesions in patients with SAPHO syndrome.
