Exercise Programs for Women at Increased Risk of or Diagnosed with Hypertensive Disorders of Pregnancy: A Scoping Review
To map the existing literature on structured exercise interventions for the prevention and management of hypertensive disorders of pregnancy (HDP) and to identify gaps for future research.
Approach:
Methodology: Conducted a scoping review in accordance with PRISMA-ScR, searching four electronic databases and two trial registries for studies evaluating structured exercise interventions in pregnant women with HDP or at high risk.
Key Findings:
Thirteen articles published between 2008 and 2025 were included.
Interventions included aerobic exercise, resistance training, combined exercise, stretching, and yoga.
Evidence was limited by heterogeneity in exercise prescription parameters and insufficient operationalization of exercise intensity.
Interpretation:
Current evidence on structured exercise interventions for HDP is limited and heterogeneous, insufficient for informing population-specific exercise prescriptions.

Substantial heterogeneity in exercise prescription parameters.
Insufficient operationalization of exercise intensity and safety monitoring.
Inadequate reporting of adherence and intervention fidelity.
Conclusion:
Future trials should adopt rigorous designs, clearly define and monitor exercise intensity, and include clinically meaningful maternal and neonatal outcomes.
