Objective:

To report a case of exceptionally large retropharyngeal lipoma (RPL) and review previously reported giant cases.

Approach:
  • Case Presentation: A 55-year-old male presented with mild dyspnea, dysphagia, neck swelling, and snoring. Imaging confirmed a giant RPL measuring 140 × 110 mm.
  • Surgical Intervention: Transcervical resection with prophylactic tracheostomy was performed, and the excised tumor weighed 240 g.
Key Findings:
  • Giant RPLs can grow significantly without symptoms, leading to variability in size at diagnosis.
  • The case highlights the efficacy of surgical treatment for giant RPL.
  • A literature review indicates a higher prevalence in males and a greater multiplicity of symptoms.
Interpretation:

Nerve integrity monitoring was considered useful for locating the recurrent laryngeal nerve during surgery.

Limitations:
  • The review lacks a comprehensive analysis of all giant RPL cases due to the rarity of the condition.
  • Individual assessment is necessary for prophylactic tracheostomy and postoperative care.
Conclusion:

Giant RPL may be included in differential diagnoses for patients with worsening dysphagia.
