Objective:

To report a case of a 76-year-old female with severe pelvic organ prolapse (POP) and concomitant rectal prolapse (RP) treated with a novel transvaginal mesh (TVM) approach that integrates the anterior rectal wall fixation.

Approach:
  • Surgical Technique: The procedure involved assembling TiLOOP Total 4 mesh into Total 6 configuration, connecting it to a trimmed TiLOOP mesh, and performing transvaginal fixation of the anterior rectal wall, which is significant for addressing both POP and RP.
Key Findings:
  • The modified TVM pelvic floor reconstruction with concurrent TVM rectopexy achieved favorable outcomes, with no recurrence or mesh-related complications observed at 18 months of follow-up.
Interpretation:

The findings suggest that combined TVM pelvic floor reconstruction with concurrent TVM rectopexy may be a feasible treatment option for selected cases of concurrent POP and RP.

Limitations:
  • This is a single case report with a limited sample size and no control group, which may affect the generalizability of the findings.
  • The follow-up period was relatively short.
Conclusion:

Further studies with larger cohorts and extended follow-up are warranted to validate the efficacy of this procedure.
