Objective:

To determine the annual prevalence of cardiovascular disease (CVD) preventive measures among individuals with HIV at very high estimated 10-year CVD risk from 2012 to 2021, focusing on the implications of these measures.

Key Findings:
  • The proportion of individuals with >10% estimated 10-year CVD risk increased from 32.4% to 52.1% between 2012 and 2021.
  • 67.4% of individuals with hypertension used antihypertensives, 55.9% with dyslipidemia used lipid-lowering drugs, and only 7.4% of smokers ceased smoking.
  • A decline in the use of antidiabetics was observed among individuals with diabetes over time, from 60.3% to 57.2%.
  • Men ≥40 years and women ≥50 years were more likely to use various preventive measures.
Interpretation:

Despite an increasing number of individuals at very high CVD risk, the uptake of preventive measures remained suboptimal, underscoring the urgent need for improved awareness and management strategies in clinical care for people with HIV.

Limitations:
  • Limited data on the effectiveness of specific preventive measures.
  • Potential bias from missing data despite efforts to include only cohorts with high data completeness.
  • Findings may not be generalizable beyond the RESPOND cohort.
Conclusion:

The findings highlight the urgent need for enhanced CVD risk management strategies among individuals living with HIV, particularly as the population ages and CVD risk increases.
