Objective:

To report the laparoscopic management of large left paratubal cysts in two patients and review existing literature on the topic.

Approach:

• Case Presentation: We report the cases of two young women who presented with pelvic pain and abdominal distension due to large left paratubal cysts. They underwent laparoscopic surgery, which provided histological confirmation and symptom relief.
• Surgical Technique: The laparoscopic approach was performed under pneumoperitoneum with a strategy to prevent spillage of cyst contents.

Key Findings:

• Laparoscopic management of large paratubal cysts is safe and feasible.
• Expertise is required to avoid cyst content spillage and preserve fertility.
• Surgical resection is necessary for diagnosis and treatment.

Interpretation:

Laparoscopic surgery is effective for managing large paratubal cysts, but careful consideration is needed regarding cyst size and potential malignancy.
