Objective:

To compare mid-to-long-term clinical outcomes of Disk Percutaneous Ablation and Extraction (DPAE) and standard Disc-FX in patients with lumbar discogenic pain and contained herniation.

Approach:
    Key Findings:
    • DPAE group had a mean VAS reduction of 5.29 ± 2.06 vs. 4.17 ± 2.24 in Disc-FX group (p < 0.001).
    • DPAE showed better improvement in ODI, higher PGIC marked improvement, and higher composite success rate (70.0% vs. 51.1%, p < 0.05).
    • Reintervention rate was lower in DPAE group (7.3%) compared to Disc-FX (14.8%), with a reduced risk (HR = 0.46, p = 0.047).
    • Complication rates were similar between groups (6.4% vs. 9.1%).
    Interpretation:

    Limitations:
    • Single-center study may limit generalizability.
    • Retrospective design may introduce selection bias.
    Conclusion:

    Future multicenter prospective studies are warranted to validate the long-term efficacy of DPAE and define optimal indications.
