To assess the diagnostic performance of diverse myocardial strain parameters derived from speckle-tracking echocardiography in identifying coronary artery disease.
Approach:
Systematic Search: Systematic searches of PubMed, Embase, Web of Science, and the Cochrane Library were conducted covering inception through September 1, 2025.
Key Findings:
Pooled STE sensitivity for CAD was 77.3% (95% CI: 73.4–80.8%) and specificity was 75.0% (95% CI: 71.5–78.1%).
Global longitudinal strain (GLS) showed sensitivity of 78.6% (95% CI: 74.5–82.2%) and specificity of 76.3% (95% CI: 72.7–79.7%).
Global area strain (GAS) had the highest sensitivity at 83.3% (95% CI: 74.5–89.5%) but moderate specificity at 72.2% (95% CI: 58.9–82.5%).
Exercise stress STE had greater sensitivity than dobutamine stress or no-stress protocols.
