Diagnostic Accuracy of Speckle Tracking Echocardiography in Identifying Coronary Artery Disease: A Systematic Review and Meta-Analysis

Objective:

To assess the diagnostic performance of diverse myocardial strain parameters derived from speckle-tracking echocardiography in identifying coronary artery disease.

Approach:
  • Systematic Search: Systematic searches of PubMed, Embase, Web of Science, and the Cochrane Library were conducted covering inception through September 1, 2025.
Key Findings:
  • Pooled STE sensitivity for CAD was 77.3% (95% CI: 73.4–80.8%) and specificity was 75.0% (95% CI: 71.5–78.1%).
  • Global longitudinal strain (GLS) showed sensitivity of 78.6% (95% CI: 74.5–82.2%) and specificity of 76.3% (95% CI: 72.7–79.7%).
  • Global area strain (GAS) had the highest sensitivity at 83.3% (95% CI: 74.5–89.5%) but moderate specificity at 72.2% (95% CI: 58.9–82.5%).
  • Exercise stress STE had greater sensitivity than dobutamine stress or no-stress protocols.
  • Deek's funnel plot indicated no significant publication bias (P = 0.202).
Interpretation:

Overall certainty of evidence was low, reflecting high risk of bias, indirectness, and imprecision.

Limitations:
  • High risk of bias in included studies may affect the reliability of the findings.
  • Substantial heterogeneity across studies limits the generalizability of the results.
  • Low overall certainty of evidence suggests caution in interpreting the findings.
Conclusion:

STE demonstrates moderate diagnostic accuracy for CAD, with GLS being the most robust parameter.
