Impact of E-Coach chronic disease management model combined with the WeChat platform on self-management ability in hypertension patients
To evaluate the impact of the E-Coach chronic disease management model integrated with WeChat on self-management capabilities in patients with hypertension.
Approach:
Study Design: Single-center randomized controlled trial involving 250 middle-aged and elderly hypertension patients.
Groups: Participants were divided into a control group (conventional management) and an intervention group (E-Coach model via WeChat).
Outcomes: Primary outcomes included CDSMS and PACIC scores; secondary outcomes included SBP, DBP, hospital visits, medication adherence, and BP control rates.
Key Findings:
Intervention group showed higher CDSMS scores in all dimensions compared to control group (P < 0.001).
PACIC scores were significantly higher in the intervention group across all dimensions (P < 0.001).
SBP and DBP levels were lower in the intervention group (P < 0.001).
Intervention group reported fewer outpatient visits, emergency visits, and hospitalizations (P < 0.001).
Higher medication adherence and BP control rates were observed in the intervention group (P < 0.001).
Interpretation:
The E-Coach chronic disease management model combined with WeChat improves self-management ability in hypertension patients and demonstrates effective chronic disease management.
Limitations:
Single-center study may limit generalizability.
Focus on short-term outcomes (6 months) may not reflect long-term effectiveness.
Conclusion:
The E-Coach chronic disease management model integrated with WeChat enhances self-management skills in hypertension patients.
