To systematically evaluate the diagnostic performance of D-dimer for screening DVT after total joint arthroplasty (TJA).
Approach:
Literature Search: Databases searched included PubMed, Web of Science, Wiley Library, CNKI, Wanfang Data, and VIP from inception to November 2025 for studies on D-dimer's diagnostic accuracy for DVT after TJA.
Study Quality Assessment: Study quality was assessed using the QUADAS-2 tool.
Statistical Analysis: A bivariate mixed-effects model was applied to pool sensitivity, specificity, PLR, NLR, and DOR, with SROC curves constructed and AUC calculated using Stata version 14.0.
Key Findings:

Interpretation:
D-dimer shows moderate diagnostic potential for screening DVT after TJA, primarily useful for ruling out DVT when results are negative.
Limitations:

Conclusion:
D-dimer should be considered as part of a risk-stratified screening strategy, ideally combined with clinical risk assessment models or specific cutoff values.
