In a comprehensive analysis of the SENIOR-RITA randomized clinical trial, researchers examined the efficacy of routine invasive strategies versus conservative management in frail patients aged 75 and older with non–ST-elevation myocardial infarction. Among the 1,446 evaluated patients, those following invasive management exhibited higher rates of cardiovascular death and nonfatal myocardial infarction compared to conservative management, particularly at higher levels of frailty. Notably, the invasive strategy failed to demonstrate significant benefits, suggesting potential harm among the frailest individuals. Lead researcher Francesca Rubino, MD, emphasizes the need for careful evaluation of treatment approaches in this vulnerable population.
