Objective:

To estimate the prevalence of premature ovarian insufficiency (POI) in women with systemic lupus erythematosus (SLE) exposed to cyclophosphamide (CYC) and explore sources of heterogeneity.

Key Findings:
  • The pooled prevalence of POI in SLE treated with CYC was 15% (95% CI 7-26%).
  • Substantial heterogeneity was observed (I2 = 85%; Cochran’s Q = 73.98, p <0.0001).
  • Prevalence decreased in studies published after 2005 (9%) compared to pre-2005 (29%).
  • Age at exposure was identified as a significant predictor of POI prevalence.
Interpretation:

The findings indicate a substantial burden of POI among women with SLE treated with CYC.

Limitations:
  • Variability in diagnostic criteria for POI across studies may affect prevalence estimates.
  • Cumulative CYC dose was not independently associated with prevalence, possibly due to lower-dose regimens adopted after 2005.
Conclusion:

Women with SLE treated with CYC have an estimated prevalence of POI of approximately 15%.
