Objective:

To evaluate and quantify the risk of postpartum hemorrhage in women with endometriosis and adenomyosis.

Approach:
  • Study Design: A prospective cohort study comparing women with endometriosis and adenomyosis to a control group without these conditions.
  • Participants: 82 women with endometriosis and adenomyosis matched with 82 women without these conditions.
  • Definition of PPH: Postpartum hemorrhage was defined as blood loss of ≥1000 mL during childbirth.
  • Data Collection: Cumulative blood loss was documented using standardized clinical methods.
Key Findings:
  • 11 women in the study group experienced postpartum hemorrhage (13%) compared to 1% in the control group.
  • The unadjusted odds ratio for postpartum hemorrhage in the EM group was 8.74 (CI:2.01-82.00, p=0.002).
  • In the adjusted analysis, the odds ratio was 7.56 (95% CI:1.39-83.70, p=0.018).
Interpretation:

Women with endometriosis and adenomyosis are more likely to experience postpartum hemorrhage compared to those without these conditions.

Limitations:
  • Small sample size.
  • Wide confidence intervals.
  • Potential misclassification due to self-reported data.
Conclusion:

The study indicates that women with endometriosis and adenomyosis are at an increased risk of postpartum hemorrhage.
