Objective:

To map the expression patterns and co-expression landscapes of Nectin-4, HER2, and Trop-2 in urothelial carcinoma (UC).

Approach:
  • Cohort Analysis: Analyzed 118 UC specimens, including matched pairs of primary tumors with adjacent normal tissues, carcinoma in situ, and lymph node metastases.
  • Expression Evaluation: Expression levels were evaluated via immunohistochemistry (IHC).
  • Statistical Analysis: Statistical analyses included the Wilcoxon signed-rank test, Mann–Whitney U test, and multivariable logistic regression.
Key Findings:
  • 90.7% of patients exhibited high expression of at least one target.
  • 56.8% showed multi-target co-expression.
  • HER2 and Trop-2 were significantly upregulated from carcinoma in situ to invasive carcinoma.
  • Nectin-4 tended to downregulate in metastases.
  • Trop-2 expression was significantly higher in the lower urinary tract compared to the upper urinary tract.
Interpretation:

The study reveals significant antigenic drift and anatomical expression biases during disease progression, advocating for dynamic, multi-site molecular profiling.

Limitations:
  • The study is retrospective and may not capture all variables influencing expression patterns.
  • The sample size may limit the generalizability of the findings.
Conclusion:

The findings provide a translational blueprint to optimize ADC sequencing and address heterogeneity-driven resistance, introducing the Δscore as a new metric for quantifying net antigenic gain.
