Objective:

To evaluate the efficacy of a 4-week vancomycin pulse-and-taper regimen compared to a 2-week pulse regimen followed by placebo in preventing recurrence of Clostridioides difficile infection (CDI).

Approach:

    • The 4-week pulse-and-taper regimen had a 74% probability of being superior to the 2-week pulse regimen followed by placebo in preventing recurrence.
    • Recurrence rates by day 56 were 15% in the pulse-and-taper group vs 18% in the placebo group.
    • Recurrence rates by day 38 were 7% in the pulse-and-taper group vs 15% in the placebo group.
    • Adverse effects were uncommon in both treatment approaches.
    Interpretation:

    The pulse-and-taper regimen may represent a safe and effective option for delaying or preventing early CDI recurrence, although the observed differences may be influenced by delayed recurrences.

    Limitations:
    • The trial was stopped early due to recruitment feasibility challenges.
    • Further evidence is needed to optimize therapy, including head-to-head trials against fidaxomicin.
    Conclusion:

    The findings suggest that a vancomycin pulse-and-taper regimen could be beneficial in managing CDI recurrences, warranting further research.
