To evaluate the effectiveness of the BioTAK score in distinguishing takotsubo syndrome from acute coronary syndrome.
Approach:
Study Design: The BioTAK score was developed using data from 3,615 patients in two prospective registries, with a focus on blood samples collected prior to coronary angiography.
Biomarkers Used: The score incorporates four biomarkers: NT-proBNP, LDL cholesterol, sLOX-1, and PAM, along with patient sex.
Validation: The score was validated in a separate cohort of 1,792 patients.
Key Findings:
BioTAK achieved an area under the curve of 0.97 in the development cohort and 0.93 in the validation cohort.
A score below 27 had 95.7% sensitivity and a negative predictive value of 99.8%.
A score of 33 or higher had 99% specificity but a positive predictive value of 59.1%.
BioTAK was noninferior to existing diagnostic scores that require more complex patient histories.
Using predefined thresholds, the score placed 89.2 percent of patients in either a low- or high-probability group.
Limitations:
The study did not assess whether BioTAK reduces angiography or improves patient outcomes.
Laboratory availability for routine assays of sLOX-1 and PAM is currently limited.
Independent validation is needed due to the small number of takotsubo syndrome cases in the cohorts.
