In the INSTANT randomized clinical trial published in JAMA, researchers found that patients with acute ischemic stroke who showed insufficient early responses to tenecteplase experienced improved functional outcomes at 90 days when treated with adjunctive intravenous tirofiban instead of placebo. The trial involved 359 patients in China and demonstrated a higher rate of excellent outcomes (64% vs. 52%) with tirofiban, though the adjusted analysis lacked significance. The study underscores tirofiban's potential as an adjunctive treatment, particularly for specific patient groups, while also noting constraints regarding generalizability and treatment timing.
